CCT clients will receive HCBS waiver services as well as CCT supports and demonstration services while enrolled in the program.  The waivers available for CCT clients are listed below.

 

Persons with Brain Injury	Community Mental Health Supports	Persons who are Elderly, Blind & Disabled (age 18-64)
Persons who are Elderly, Blind & Disabled (age 65+)	Supported Living Services	Persons with Developmental Disabilities

 

 

Colorado Choice Transitions has 14 demonstration services for client to access that complement the HCBS waivers.  Demonstration services may vary by waiver.
