New look for Medical Identification Cards
Beginning June 1, 2010 the Department began issuing Medical Identification Cards with a new look. The new cards are bilingual - English and Spanish - and more informative. These cards do not replace cards issued before June 1, 2010, therefore, please accept both versions. To see an example of the new look, please click here.
