How to Apply

Have you applied online but didn't receive a denial or approval? Go to My Application Status.

The Colorado Department of Health Care Policy and Financing administers Medicaid, Child Health Plan Plus, and the Colorado Indigent Care Program for Coloradans who qualify.

 

NOTE:  We are experiencing extremely high call volume with wait times of up to an hour. Please be patient.

 

  • If your question is about the status of your Medicaid application you should call 1-800-359-1991.    
  • If your question is related to private health insurance available through Connect for Health Colorado, you should call 1-855-752-6749.

  • Medicaid is free or low cost public health insurance for Coloradans who qualify. Medicaid can cover your doctor visits, emergency care, preventative care such as screenings and immunizations, and other procedures and treatments. You may also qualify for Long-Term Care Medicaid which can include nursing facility care and Home and Community-Based Services where health care providers deliver care in your home.
  • Child Health Plan Plus (CHP+) is low-cost public health insurance for Colorado's uninsured children and pregnant women who earn too much to qualify for Medicaid, but cannot afford private insurance.
  • Colorado Indigent Care Program (CICP) provides discounted health services through qualified providers to Colorado residents, migrant workers and legal immigrants who are uninsured or underinsured. CICP is not a health insurance program. Services are restricted to participating hospitals and clinics throughout the state. Click here to find out how to apply for CICP.

Beginning in January 2014, Medicaid will cover more Coloradans. To find out if you qualify for free or low cost health insurance, or for financial assistance to help you buy private health insurance through the Connect for Health Colorado marketplace, apply online, in person, by mail, or by phone.

Application for Health Coverage and Help Paying Costs.  This application is for health care coverage only.

 

Application for Public Assistance. This application is for health care coverage, food assistance, and cash programs.

The Affidavit to Establish Identify form can be used to establish identify of a person who meets one of the  following:

·         Child age 0-15 for whom no other acceptable identity documents such as: clinic, doctor, hospital, or school records are available or;

·         Individual age 16-18 for whom no other acceptable identity documents are available and in instances when school ID cards and drivers' licenses are not available until the individual reaches that age or;

·         A disabled individual in an institutional care facility, only if: No other evidence of identity is available to the individual and it must be signed by the residential care facility director or administrator on behalf of the institutionalized individual in the facility.

Immunization (shots) Registry Notice

PLEASE NOTE:  The following refers to questions about Immunizations/CIIS only. If you have general Medicaid, eligibility or application questions, you should contact the Customer Contact Center at 1-800-221-3943 or email customer.service@hcpf.state.co.us.

Colorado has an immunization registry, a central place for health care providers to keep track of immunizations.  This helps providers know what shots you/your child have had and which ones are still needed. The information in the registry is completely confidential.

Under Colorado law, you can choose to opt out of participating in the registry.  If you opt out, you/your child's immunization history will not be in the registry, and you will need to keep track of this information. You can decide to opt out of the registry at any time.  For more information and an opt-out form, click here
