In-Home Support Services
Case managers can terminate Eligible Consumers from the program for one or more of the following reasons:
- Loss of Medicaid eligibility and loss of HCBS-EBD eligibility or Childrens HCBS Waiver eligibility;
- Progression of a medical condition that causes an unsafe situation for the Eligible Consumer, as determined by the primary care or treating physician; or
- Refusal to designate an Authorized Representative if the Eligible Consumer is unable to direct his/her own care, as determined by the primary care or treating physician.
