How do I ensure that I continue to receive Medicaid or CHP+ benefits?  

 

Turn in your annual re-enrollment/re-determination packet to your worker before the due date.  To find out when the packet is due, go to the Colorado PEAK Web site and click on the "Check My Benefits" button, then click on the "My Benefits" tab. The date your packet is due is found in the "My redeterminations" section.
