Turn in your annual re-enrollment/re-determination packet to your worker before the due date. To find out when the packet is due, visit Colorado PEAK and click on "Check My Benefits," then "My Benefits" tab. The date your packet is due is found in the "My Redeterminations" section.
I am a Medicaid or CHP+ Prenatal client, how do I report the birth of my newborn?
