The Office of Administrative Courts has a wide variety of forms to assist parties in providing the Court information we need to process your appeal or hearing request. Depending on the type of case, some forms are required.
Unless otherwise directed, you should print out and complete the relevant form, and send via US mail, hand delivery or fax to:

If you have a Workers' Compensation Hearing, please go to the Workers' Compensation Forms page. Please note, depending on your location, you may need to send the relevant form to one of the OAC's regional offices for processing.
If you are appealing a denial or reduction of benefits (for example: Food Stamps, Colorado Works, Medicaid, etc) or are appealing a county finding of child abuse or neglect, please go to the General Services Forms page.
