PPO Options - both options use the UnitedHealthcare network of providers.
- PPO Co-Pay Choice Plus - uses co-pays for most services
- PPO HDHP Definity - a high-deductible health plan, eligible for opening/contributing to health savings account (HSA); most services are subject to deductible, except preventive.
HMO Options - both options use the Kaiser Permanente network of providers / facilites; available in three geographic areas, Denver/Boulder, parts of southern Colorado, and parts of northern Colorado.
