Policy Number for State-paid Basic and Employee-paid Supplemental Life/AD& D Insurance - 33780-G

The State-paid Basic Life and Accidental Death and Dismemberment (AD&D) life insurance provides $50,000 of coverage at no cost to employees. For employees on leave of absence, any increased benefit will not become effective until the employee has worked one day.

Additionally, you may apply for Supplemental Life and AD&D (Accidental Death and Dismemberment) coverage. Coverage is provided by Minnesota Life.
