Please select the appropriate form below, complete, and submit it to DOH so that we can properly report to HUD the set-up or completion of your project:

• Homebuyer/Homeowner Rehab Completion Report
• Rental Project Completion Report
• Tenant-Based Rental Assistance Set-up Report
• Multi-Address Homebuyer/Homeowner Rehab Completion Form

Completion instructions are included with each form.

Please use the following HUD forms upon completion of your project to report Section 3 and MBE/WBE

• Section 3 - Form 60002  Summary Report, Economic Opportunities for Low- and Very Low-Income Persons
• Contract and Subcontract Activity - Section 3 - 2516 MBE/WBE
