Certain facilities are required to complete and return a self-certification checklist when asked to do so by the Division. These checklists are intended to build awareness within the regulated community of applicable regulatory requirments by asking facilities to periodically review the requirements and certify they are in compliance.

Forms must be completed by an employee that is directly involved with waste management at your facility.

Please use the links provided in the checklist. DO NOT use your browser "Back" button as you will be taken out of the checklist and your answers will not be saved.

You must have the free Acrobat Reader Version 8.0 or higher in order to complete these forms. If you don't already have this free reader, you may download it from Adobe now.

If you have more than one facility, complete the certification checklist for the facility noted on the postcard or other notification you received. Please print a copy of your completed form for your records before hitting the "Submit" button. Once you hit the Submit button, you will automatically be re-directed to a related web page and will not be able to print your form. Your certification is not complete until you have hit the Submit button on the form.

If you need to make changes to your notification

If you need to make any changes to your hazardous waste notification form, such as a change in ownership, name of facility, contact information, change in location, or change in your generator status, please complete and return an updated Colorado Hazardous Waste Notification Form pdf file .
