Certain facilities are required to complete and return a self-certification checklist when asked to do so by the Division. These checklists are intended to build awareness within the regulated community of applicable regulatory requirments by asking facilities to periodically review the requirements and certify they are in compliance.
Forms must be completed by an employee that is directly involved with waste management at your facility.
Please use the links provided in the checklist. DO NOT use your browser "Back" button as you will be taken out of the checklist and your answers will not be saved.
If you have more than one facility, complete the certification checklist for the facility noted on the postcard or other notification you received. Please print a copy of your completed form for your records before hitting the "Submit" button. Once you hit the Submit button, you will automatically be re-directed to a related web page and will not be able to print your form. Your certification is not complete until you have hit the Submit button on the form.
Some of these forms are PDF forms that work best with current versions of Internet Explorer and either Adobe Acrobat or the free Adobe Reader. They will not work with Chrome or some Apple systems. You must have the free Acrobat Reader Version 8.0 or higher in order to complete the PDF forms. If you don't already have this free reader, you may download it from Adobe now.
