The regulations require transporters of waste trap grease, facilities that collect, store, process or dispose of waste grease, and individuals who collect or transport waste grease for personal use register with the the department, and renew their registration by June 1 of each year.
The following forms work best with an Internet Exporer 8 or newer browser. Please allow 2-4 weeks to receive your certificate of registration and decals, if applicable.
Waste grease transporters and facilities are required to submit a waste grease annual report to the department by February 1 of each year.
