Forms and Applications

 

These online forms work best with current versions of Internet Explorer and either Adobe Acrobat or the free Adobe Reader. Most will not work with Chrome or some Apple systems.

 

 

 

 

 

Registration Requirements

 

The regulations require transporters of waste trap grease, facilities that collect, store, process or dispose of waste grease, and individuals who collect or transport waste grease for personal use (other than for use as biofuel) register with the Colorado Department of Public Health and Environment (the department). Waste grease transporters, facilities and personal users are required to renew their registration by June 1 of each year.

 

The following forms work best with an Internet Exporer 8 or newer browser. Please hit the submit button only once as it may take a few moments to process your submission. Please allow 2-4 weeks to receive your certificate of registration and decals, if applicable.

 

 

  • WG-2 Waste Grease Manifest Form pdf file  (Print only) - The Uniform Waste Grease Manifest form can be downloaded using the link above or be obtained from the department. To obtain copies of the manifest from the department, email your name, mailing address, Transporter registration number and number of forms requested to cdphe.hmwastegrease@state.co.us . Please allow 4-6 weeks for delivery.

     

Some Transporters may wish to pre-complete certain portions of the manifest form to include information about themselves, frequent customers and/or destination facilities. You may download and save a blank copy of the form to your hard drive using Adobe Acrobat or the free Adobe Reader Version XI or higher. Once saved on your hard drive, you should be able to open the form to add your information, then save it as a new file. Be sure to print multiple copies of the partially completed form so that you have a copy for the generator and destination facility.

 

 

 

Annual Reporting

 

Waste grease transporters and facilities are required to submit a waste grease annual report to the department by February 1 of each year.

Registration / Registration Renewal

 

The regulations require haulers of waste tires and facilities that generate, collect, store, process and / or use waste tires register with the Colorado Department of Public Health and Environment.  For companies with more than one location, each location must be registered separately. Waste tire haulers are required to renew their registration by February 1 of each year.  Waste tire facilities must renew their registration every three years.

 

 

  

  • WT-3 Surety Bond Form pdf file  - You may download and save a blank copy of this form to your hard drive using Adobe Acrobat or the free Adobe Reader Version XI or higher. Once saved on your hard drive, you should be able to open the form to add your information, then save it as a new file. 

 

Manifests

 

All waste tire haulers are required to use the Uniform Waste Tire Manifest for pickup and/or delivery of waste tires in Colorado.
