It is important that you keep track of the shots that you/your child have received.
Immunization registries like the Colorado Immunization Information System, or CIIS, are one of the best tools we can use to ensure that we all receive the shots we need in order to be protected from harmful diseases. As families and individuals move in and out of public and private healthcare systems across Colorado, you and your healthcare provider can use CIIS as a powerful tool to ensure that you/your child receive the immunizations you need to stay healthy and happy.

Also known as Colorado’s immunization registry, the Colorado Immunization Information System (CIIS) is a confidential, population-based, computerized system that collects and consolidates vaccination data for Coloradans of all ages from a variety of sources. CIIS is a program within the Colorado Immunization Section at the Colorado Department of Public Health and Environment (CDPHE). CIIS helps healthcare providers, schools, childcare centers, universities, and individuals keep track of the shots that they and/or their children have received.

Vaccines protect your/your child’s health against many dangerous, preventable diseases and are required by law for children in attendance at schools, childcare facilities, camps, and universities. But it can be difficult to keep up with your/your child’s shot record, especially if you move or change healthcare providers. Keeping all the shots in one place, and on one record, helps ensure that you and your child get the complete schedule of immunizations – no more and no less. |

The shots that you/your child have already received and any shots that you/your child will receive in the future will be entered into CIIS. Other information, including parent’s/guardian’s name (for individuals under 19 years of age), gender, Vaccines For Children eligibility status (for individuals under 19 years of age), date of birth and the office where each shot is given will be included in the registry to be sure that your/your child’s record is accurate and cannot be confused with another patient’s record. Your address will also be included in the system to allow your healthcare provider to send you reminders when you/your child are due for an immunization. Yes. You may request copies of your/your child’s immunization record by completing the Request to Release Immunization Record Form. This information can be accessed through the below FAQ section "How do I Request an immunization record from CIIS?". Forms must be notarized and submitted to CIIS with a legible photocopy of your driver’s license or other secure and verifiable document. Prior to completing the form, please contact CIIS to confirm that the requested record is in CIIS. Please note that CIIS is a voluntary system and, as such, does not contain complete immunization records for ALL Coloradans. |

A patient may request copies of their immunization record by completing a Request to Release Immunization Record Form (accessed in various languages through the link below), having it notarized, and submitting it to the Colorado Immunization Information System, along with a legible photocopy of their driver's license or other secure and verifiable document. Is your/your child’s immunization record protected? In order for your/your child’s shot record to be included in the Colorado Immunization Information System, your healthcare provider must enroll and participate in CIIS.
The enrollment process is free, fast and easy. Participation in CIIS by your healthcare provider ensures that your/your child’s immunization record is secure, up-to-date and easily accessible. Ask your healthcare provider if they currently participate in CIIS.

Immunization records are confidential and personal medical information. The Colorado Immunization Registry Act of 2007 designated the Colorado Immunization Information System as a lifelong immunization record tracking system. CIIS has signed agreements with all participating sites that are authorized to provide information to or access information from the immunization registry. CIIS and all persons and entities that access immunization records are required to maintain the confidentiality of those records. The CIIS Confidentiality Policy applies to all individually identifiable information in all formats, including paper-based and electronic records.
*Please note: you cannot get your/your child’s shot record directly from CIIS; you must contact your healthcare provider for your/your child’s record.
Information in CIIS can only be released to:
Any person who releases or makes confidential immunization records public in any unauthorized manner commits a class 1 misdemeanor and upon conviction thereof, shall be punished by 6-18 months in jail or a fine of $500 to $5000, or both. The unauthorized release of each record shall constitute a separate offense. The CIIS Confidentiality Policy and CIIS Security Policy are reviewed and potentially revised at least annually. You may at any time choose to have your/your child’s shots excluded from the Colorado Immunization Information System.

Under Colorado law you have the option to exclude your/your child’s immunization information from the Colorado Immunization Information System at any time. If you change your mind, you can always have your healthcare provider re-enter your/your child’s immunization record into CIIS at a later time. If you choose not to participate in CIIS, you are responsible for keeping track of your/your child’s shot record.
