We mail Form 1099G (Certain Government Payments) in mid-January to anyone who received an unemployment benefit payment(s) during the previous calendar year.
If you did not receive Form 1099G in the mail, please wait until mid-February before requesting a duplicate copy of the form.
If your Form 1099G is mailed to an address other than your current address and a current forwarding order is on file, the U.S. Postal Service will forward the Form 1099G . Otherwise we keep the returned forms and automatically remail it once you submit an address change.
