1099G Information

The Colorado Department of Labor and Employment, Unemployment Insurance (UI) Program will be mailing the income tax Form 1099G, Certain Government Payments, in mid-January, to all claimants that were issued UI benefit payments between January 1 and December 31 of the current tax-filing year. If you do not receive Form 1099G in the mail, contact the UI Customer Contact Center, but please allow four weeks from mid-January before requesting a duplicate copy of the form.

Overview and Address Change

Form 1099G, Certain Government Payments, is mailed in January to anyone who received an unemployment insurance (UI) benefits payment during the previous calendar year.
If your Form 1099G is mailed to an address other than your current address, the U.S. Postal Service will forward the Form 1099G if a current forwarding order is on file. Otherwise, UI Operations keeps the returned Form 1099G and automatically remails it once you submit an address change.

NOTE: To request a duplicate of Form 1099G for any year, follow the instructions under "Request a Duplicate of a Past Year's Form 1099G" below.

If your address has changed since you last provided your address to UI Operations and you need to provide your new address for delivery of Form 1099G, you may use one of the following methods to notify UI Operations:

Log in to your MyUI Claimant account. Click on the My Profile tab and then click on the edit profile link in the top, right corner of the page. This page allows you to change your address. If you do not have a MyUI Claimant account, you may register for one at MyUI Claimant.

Call the Colorado Unemployment Benefits Line (CUBLine) to submit your address change. You must have an active personal identification number (PIN) to use this option. Call CUBLine at 303-813-2800 (Denver-metro area) or 1-888-550-2800 (outside Denver-metro area). Follow the prompts to report an address change. CUBLine is available 24 hours a day, 7 days a week.

E-mail your address change. You must provide specific information for proper identification. ­You will receive an e-mail confirmation that your request has been processed and your address has been changed. Please follow the instructions below:

  • Address your e-mail to UI.AddressChange@state.co.us.
  • Enter "1099G" in the Subject box to speed up processing.
  • In the body of the e-mail, include:
    • Your name.
    • The last four digits of your social security number.
    • Your previous address.
    • Your previous telephone number.
    • Your current address.
    • Your current telephone number.
    • Your birth date.
    • The name of at least one employer that was part of your UI claim.
  • Send the e-mail.

Mail or fax a written request for an address change. Please follow the instructions below:

  • Write:
    • Your name.
    • The last four digits of your social security number.
    • Your previous address.
    • Your current address.
    • Your current telephone number.
    • Your birth date.
    • The name of at least one employer that was part of your UI claim.
  • Sign your request.
  • Fax your request to 303-318-9011, attention: 1099.

For Those Having an Overpayment of Unemployment Benefits

Your 1099G shows the entire amount we paid you in Benefits, even though you are paying back those benefits. Repayments are not subtracted from that entire amount. Your federal income tax form has a place to report the amount you repaid. Check the instructions on that form to see how to handle this situation. We cannot help you (this could be considered giving you advice about your federal taxes.) Check with the IRS if you are not sure what to do.

For Those Who Do Not Recall Receiving Unemployment Benefits

Remember that we send your benefits to the debit card (CAP card) that Chase sent you when you first signed up for unemployment. Contact Chase Customer Service at 1-866-316-3925 to find out the balance on your debit card. It is possible that we sent the benefits.
Request a Duplicate of a Past Year's Form 1099G

To request a duplicate of a past year's Form 1099G:
  • Send an e-mail to UI.AddressChange@state.co.us. Include your name, telephone number, and social security number with the request.

  • Call the Customer Contact Center at 303-318-9000 (Denver-metro area) or 1-800-388-5515 (outside Denver-metro area).

NOTE: If you need to change your address, please submit the address change before requesting a duplicate Form 1099G. Follow the instructions in the section above for submitting an address change.  If you submit your request for a duplicate via e-mail, an address change may be submitted in the same e-mail.
