CDHS Policy I-2.4, Separation Notification established the procedure for advising impacted CDHS Offices of an employee's notice to transfer or separate from the Department in order to ensure consistent notification and timely removal of the employee from the payroll so that overpayment does not occur.
Appointing Authorities or their designee are responsible for advising certain offices within CDHS of an employee's transfer to another position within CDHS, transfer to another State agency or separation from CDHS due to resignation, retirement or other reason(s). Additionally, appointing authorities or their designee are also responsible for retrieving the CDHS Identification Card, state equipment and property. The Separation Notification form provides a checklist for items and additional paperwork required. Fax the completed form to the Offices identified at the bottom of the form.
