Reporting Reminders:
- All surveys must be annonymous.
- Receiving services may not be contingent upon completing a survey.
- Only report complete surveys to DVP.
- For example, if a total of 10 surveys were turned in, but 2 of the surveys did not answer the outcome question regarding enhanced safety strategies, you would report the following to DVP:
- x of 10 reported that they had increased their knowledge of community resources.
- x of 8 reported that they had enhanced their safety strategies.
