Facility Inquiry form is to be used by all facilities licensed by CDHS, Division of Child Care. Please ensure that only one license number is listed on the form and that it is the license number for the facility at which the individual works or is identified as working at. This includes those employees who float between licensed facilities. Please use the same license number that was used on the fingerprint card when the card was submitted to the Colorado Bureau of Investigation (CBI).
For Family Child Care Homes
Background Investigation Unit Facility Inquiry Form: This form must be completed for each adult NOT LIVING in the provider’s home that provides care for children, which includes a permanent substitute(s) and the second (2nd) provider for a large family child care home. The completed form must be accompanied by a SEPARATE check or money order for $33.00, made payable to CDHS, BIU Records and Reports. Include the completed form and check with your application packet.
For Child Care Centers
Background Investigation Unit Facility Inquiry Form: This form must be completed for each employee at the child care center, which includes substitute(s). The completed form must be accompanied by a SEPARATE check or money order for $33.00, made payable to CDHS, BIU Records and Reports. Include the completed form and check with your application packet.
For Day Treatment, Residential Child Care Facility (RCCF), Psychiatric Residential Treatment Facility (PRTF), Therapeutic Residential Child Care Facility (TRCCF), Adoption and Foster Care
Background Investigation Unit Facility Inquiry Form: This form must be completed for all employees of the faciity or agency. The completed form must be accompanied by a SEPARATE check or money order for $33.00, made payable to CDHS, BIU Records and Reports. Include the completed form and check with your application packet.
FBI and CBI Fingerprint fees: Please submit a check with the fingerprint cards to the Colorado Bureau of Investigation, 690 Kipling, Suite 300, Lakewood, CO 80215. NOTE: Please maintain a copy of the final fingerprint cards for your personnel files. See additional information regarding these fees below.

Each owner, employee, who is eighteen (18) years of age or older, and newly hired employee who is eighteen (18) years of age or older, of a facility or agency must submit to CBI a complete set of fingerprints to obtain any criminal record held by the CBI. Payment of the $17.50 fee for the criminal record check is the responsiblity of the individual being checked or the facility or agency. The results of the criminal record check must be maintained at the home, center, facility, or agency and must be available for review upon request by a licnesing specialist.
Any adult volunteer, working as a staff member to meet the required staff child ratio or staff qualifications, who works 14 days or more in a calendar year, must submit to CBI a complete set of fingerprints taken by a qualified law enforcement agency to obtain a criminal record check. The results of the criminal record check must be maintained at the facility or agency and must be available for inspection by a licnesing specialist.
For Resident Children's Camps
Only in the case of a children's resident camp or school-age child care center, out-of-state persons employed in a temporary capacity for less than ninety (90) days are not required to be fingerprinted to obtain a criminal record check. Each person exempted from fingerprinting and being checked with the state Department's automated system must sign a statement which affirmatively states that she/he has not been convicted of any charge of child abuse or neglect, unlawful sexual offense, or any felony.
Prospective employers of such exempted persons shall conduct reference checks of the prospective employees in order to verify previous work history and shall conduct personal interviews with each such prospective employee.

House Bill 11-1145, passed in the 2011 session of the Colorado Legislature, mandates that all employees hired on or after August 10, 2011 at any licensed child care facility, except summer camps operating for less than 90 calendar days, shall complete a FBI background check in addition to the CBI, TRAILS, and ICON background checks currently required by law. The additional fee for a FBI background check is $22.00. Providers or prospective employees will submit their fingerprint card through the normal procedure, but will attach the additional fee.

House Bill 11-1102 passed in the 2011 session of the Colorado Legislature, creates limited portability of criminal background checks in limited circumstances, which became effective on August 10, 2011.
