Sample Collection Form
This form is used to accompany a voucher specimen (plant sample) for verification and entered into the state database if identified as anAlert List species. Please be sure to complete the form as thoroughly as possible and follow the voucher specimen instructions carefully. The more information that we have, the better equipped we will be to respond to positively identified alert species.
