The following forms are available for the Metrology Laboratory:

Work Request Form Guidelines
To provide the best service possible, the Colorado Metrology Laboratory recommends completion of the work request form before equipment will be processed for testing.  (The Work Request Form is also available in pdf format.)

  • Please include a copy of the completed form with all shipments.  Failure to include the completed form will delay processing until the form is received.
     
  • Ensure items are clean and suitable for testing or additional fees will apply.  Please see Standard Condition/Cleaning for more information.
  • Ship items to Measurement Standards, 3125 Wyandot Street, Denver, CO 80211.

  • For large mass (50 lb - 5,000 lb & weight carts)

    • Deliveries and pickups are limited to Fridays only, unless pre-approved by a lab metrologist.
    • Standards must be delivered on an open-bed truck or a truck with a lift-gate.  An overhead hoist is available.
    • Truck maximum length is 45 feet (including the cab), due to limited street access.
    • Any deviation from the above guidelines, the shipment will be refused and sent back to the customer.

Request a Reminder Postcard Form
To better serve our legal customers (scale companies and law enforcement agencies), we automatically provide a free "reminder postcard" service.  We send out this reminder approximately 1 month prior to when the standards are due for recalibration (2 months for large mass). 

We also offer this reminder system by request for private companies who require their standards to be done on a timed basis according to their management system requirements. 

Would you like to be notified when your standards are due for recalibration?  Just fill out and submit the request for a reminder postcard form and a reminder will be sent to you approximately 1 month prior to the calibration due-date.

This is a great and FREE way to gently remind you that your standards are due to come back in for calibration.
